When you mean "damaging anything" I'm assuming you mean the pages you want to use? Just x-acto them out or straight pull-em if the paper is sturdy enough, or rip a few extra pages along with it so there's a bit of buffer where your grip is. Rough edges in the spine don't matter.
I try to use covers when possible, unless they just have absolutely nothing to do with the content, but even then, I've used them to help create divisions between the ending of whatever was prior in the bind.
Any rule you try to come up with, I guarantee you'll find yourself breaking it the next time 'round.
