Only method that you can't do is smyth sewing.
Regarding the cover, it depends on if it actually represents anything from the pages included. If I'm only binding the back up stories, but the cover image is based mainly on the lead story, then no cover.
As for the rest, treat it like a TPB, and we have plenty of topics regarding that. Pull at the spine, use xacto knife...
