It depends on whether the cover is flush with the edge or not. If it's not, then it may be okay even with trimming. My Ultimates Omnibus which was trimmed had the massive internal gate-fold in Ultimates 2 #13 but LBC didn't have to do any extra folding because the gatefold was already pre-folded far away enough (~1/8 inch) from the edge.

Check the FAQ for picture of an untrimmed book. Generally, I like my books trimmed even if it may mean a little bit of art loss because the rounded spine makes the book look less professional untrimmed IMO. LBC has left a few books untrimmed though. I think James Friel left his Xenozoic Tales book untrimmed on the outer edge because of the disparity in issue sizes.

You could have the book handsewn although I think that would only work for the floppies and not the PF books.
